Upon receipt of application, the clinic will contact you by phone for an intake and eligibility appointment.
In order to qualify for services provided at the clinic, new patients must meet federal poverty guidelines. Please bring photo ID and proof of income (last year tax return, W-2, pay stubs social security disability letter or Medicaid denial letter.
Our Health Clinic provides screenings for adults ages 18 through 64 with chronic health conditions – arthritis, asthma, cancer, COPD, emphysema, diabetes, epilepsy, depression, heart disease, hypertension, and obesity. When possible, physicians may refer patients to our partners for specialty care. No narcotics are on the premises nor will we write prescriptions for any narcotic or pain medications.
Our Dental Clinic is available for patients 18 years of age and older.
Our Mental Health Clinic provides psychotherapy, counseling and individual therapy for diagnosed mental illnesses. No alcohol or drug abuse counseling provided.
Important Message: The Clinic remains committed to providing access to free healthcare in our community. As always, we appreciate any nominal amount of donation during your appointment. Importantly, no one will be turned away due to inability to donate.
A Friendly but Important Reminder: We are not an Emergency Clinic. Therefore patients experiencing any of the following conditions should go immediately to the nearest EMERGENCY FACILITY: chest pain or stroke symptoms (sudden weakness to one side of the body, sudden loss of vision in one or both eyes, slurred speech, sudden headache for no obvious reason).
We are not a walk-in clinic. Patients must have appointments.
